Chlamydia is a common bacterial sexually transmitted infection, affecting around 1 in 10 young men and women and can cause serious health problems if left untreated.
You can only receive your free Chlamydia test kit by post if you live in the Dorset area, and you are under 25.
Completing this form is giving us permission to send you a postal Chlamydia Screening Pack.
Please note: Although we send this package out in a plain plastic envelope to the address given, we cannot guarantee it not being opened by a third party and cannot be held responsible for any issues that may arise from this.

We will contact you with your results by letter, phone, text or email. We aim to contact you with your result no more than two weeks after you have taken the test. We will not contact anyone else with your result unless you wish us to.
